NDIS Service Agreement Terms & Conditions
The Provider (Next Challenge) and Client agree to the following terms and conditions:
- To be respectful and polite to each other.
- To ensure communication is open and honest in order to be achieving the best possible outcome for the client.
- To address any concerns with therapy or changes to the client as soon as possible.
- To notify each other of any changes that may impact on the client.
- To inform each other, with more than 24 hours’ notice, if an appointment cannot be kept.
- To discuss any changes that may need to be made to the service agreement (including the desire to end the service agreement).
